Registration Forms

LDS Church Permission and Medical Release Form - 

Form will need to be printed and turned in on May 31st at the Sunday evening meeting or saved as PDF and emailed to davidhalljr@gmail.com.  As a matter of convenience we have provided two options for filling out the form.

1) Partially filled out PDF form with Event Details already filled in.  You just need to fill in the remaining fields and print or save as PDF and then email.

OR 

2) BLANK form, fill in all the fields from scratch

What to enter for Event Details in the Blank form version:

Event: 2026 Young Men Camp & High Adventure

Dates of event: June 22nd – 27th

Describe event and activities:  Travel to Kentucky and participate in climbing, caving and various other activities in the area.
